他不是放棄生命 只是求個好死
生病往往不是一個人的事情，而是一個家的事情，有時候癌末病人早知道自己時日不多，治療只是徒然，勉強治療、痛苦的活著不是為了自己，而是為了別人。在癌症病房擔任心理師的江珈瑋曾遇過口腔癌末期的病人，簽署不實行心肺復甦術，卻遲遲不敢讓家人知道，怕家人以為自己放棄生命，其實他只是希望在生命的最後，能一路好走。
不論在哪個時代、什麼背景、哪種出身，誰都想好好離開這個世界的。「我其實不怕死亡，因為那是人生的必經過程，但是最怕的是要死死不了，不得好死」，這句話多麼貼切，且這已經不是第一次聽到末期病人這樣告訴我了。
勇叔也是其中一位，他不斷地說，「人活著，就是求『好死』二字的心願而已。」
勇叔初診斷口腔癌，經過三年的化學及放射線治療，療程告一個段落，這次，眉頭深鎖的勇叔走進會談室，一副沒有力氣的模樣，我輕聲問候低著頭的他，「勇叔，你還好嗎？」
他語氣沉重地說，「醫生說我腦部轉移了」，我陪著靜默幾秒鐘後，他接著說，「我內心有點不安。」
內心最不安是什麼？他相當沮喪地說，「妳知道嗎？其實我很擔心自己到時候會整個人癱著，失去意識，什麼事情也不能做，到最後連行動能力都沒有，然後完全失去意識，有種不得好死的感覺。我不想要過這種日子，那真的是一點意思都沒有。」
他哭著告訴我，「我真的覺得自己很沒用，什麼都沒辦法做，好像在等死。」我傾聽著，這也許是勇叔至今都沒有對人說出口的情緒。
我陪伴著他討論並正視死亡這個議題，他的情緒開始慢慢轉變，「現在轉移的是腦部，其實我也知道多想沒有用。如果最後真的無法好死，好像也不是我能決定的，不過我倒是有簽署不實施心肺復甦術（DNR），我覺得這是我要的死亡方式。然後政府好像開始推動《自主權利法》？」
我點點頭，並跟他解釋了《自主權利法》。人生許多遭遇，會讓我們覺得自己沒有選擇，雖然這些遭遇不是我們可以控制的，但我們依然會想要拿回一些自主權。
他若有所思後說，「其實我是一個很有自己想法的人，我簽了DNR，但還不知道如何跟家人說，我很怕他們誤以為我是放棄不想活，其實我怎麼可能不想活，能活一天是一天啊，現在我都這樣想著。我只是決定不要痛苦地死去，這很重要。」
「嗯？」我眼睛望著他，期待他繼續說下去，「對，我比較擔心的是這個，一想到我哥他們一直提醒我絕對不能放棄、要堅持到最後，一直以來替我擔心這個、擔心那個，我真的很有心理壓力，也就很難跟他們說。」
「不過，我最後還是會跟他們說，因為我真的想好死一點，也比較安心。」他說跟我談完後，心理更確定自己想要的選擇是什麼，會再努力向家人傳達自己的決定。
